Camper Forms

The online “Camper Information” (includes basic health info) is required for ALL TIC campers and must be completed and submitted through your online account.

Additionally, please complete the forms below that applies to your camper and their camp location. These forms can be submitted via upload, scan/email, fax or mail:

Campers will not be allowed to participate at camp without the required forms.

TIC-VA Campus: St. John Academy in McLean

  • Commonwealth of VA School Entrance Health Form: Required for ALL TIC-VA campers. For new TIC-VA campers, you can submit a copy of an existing health form that you or your child’s school has on file. This form MUST be signed and dated by a physician and submitted at least one week prior to the camper’s first session to include the most current IMMUNIZATION RECORDS.(Previously enrolled campers who have a COMPLETED health form on file need only provide updates.)
  • Proof of Identification: VA licensing standards require first time TIC-VA campers (since 2014) to provide proof of identity. Acceptable forms of identification include certified copy of birth certificate, birth registration card, passport or record from a public school in Virginia (original or carbon copy). If you would prefer not to send, you must bring a copy on the first day of camp for us to verify.
  • VA Medication Authorization Form: For TIC-VA campers who will need to bring prescription and/or non-prescription medications to camp. This form does NOT require a physician signature. If you prefer, you can bring this form, along with your child’s medication, on the first day of camp.
  • Permission to Walk/Bike Home: This online form ONLY needs to be completed if your child will walk or bike home after camp but MUST be completed through your online account.

TIC-MD Campus: Connelly School of the Holy Child in Potomac

  • MD Med Auth and Action Plan Form: For TIC-MD campers who will need to bring prescription and/or non-prescription medications to camp. This form REQUIRES a physician signature. If you prefer, you can bring this form, along with your child’s medication, on the first day of camp.
  • MD Immunization Certification Form: For TIC-MD campers who reside OUTSIDE of the United States.
  • Permission to Walk/Bike Home: This online form ONLY needs to be completed if your child will walk or bike home after camp but MUST be completed through your online account.
  • MD 2018 Shuttle Bus Permission Form: For TIC-MD campers enrolled in the MD Shuttle Bus. NOTE: The shuttle bus requires an additional fee and can be added when you enroll. For more information click here.

TIC-DC Campus: GDS High School in Northwest DC

  • DC Medication Authorization Form: For TIC-DC campers who will need to bring prescription and/or non-prescription medications to camp. This form does NOT require a physician signature. If you prefer, you can bring this form, along with your child’s medication, on the first day of camp.
  • TIC Immunization Form: For TIC-DC campers who reside OUTSIDE of the United States.
