Camper Forms

Below are the forms to be completed online or sent in by mail, fax, or email before June 1. PLEASE find your TIC Site and fill out the relevant forms to your location. Campers will not be allowed to participate at camp without the required forms.

 

TIC-VA Campus in McLean

(St. John Academy)

 

  1. The “Health Information” must be completed in your online account.
  2. VA State School Entrance Health Form is required for ALL TIC-VA campers and may be downloaded from the link above. For new TIC-VA campers, you can submit a copy of an existing health form that you or your child’s school has on file. This form MUST be signed and dated by a physician and submitted at least one week prior to the camper’s first session to include the most current IMMUNIZATION RECORDS. (Previously enrolled campers who have a COMPLETED health form on file need only provide updates.)
  3. Proof of Identification is one of the following documents which needs to be faxed, scan/emailed or mailed to our office. If you would prefer not to send, you must bring a copy on the first day of camp for us to verify. Please contact the Camp Office with any questions. Certified Copy of child’s birth certificate, Birth Registration Card, Passport, Record from a public school in Virginia. (original or carbon copy)
