Staff Assistant Paperwork 2021

Follow the instructions and download your required paperwork, then upload the completed copies to the CampBrain portal. The health form is already in CampBrain and can be filled out when you sign in!

Paperwork is due at least 1 week prior to your session!

Voluntary Disclosure Form- Required for ALL STAFF ASSISTANTS

W-2 Tax Form- Required for any Staff Assistant working more than One Session (Please note- this document is crazy long. You just need to fill out one of the copies of the half-page form, don’t worry about the others)

Work Permit Form- Required for any DC Staff Assistant working more than One Session (NOT required for VA)- (You can ignore that it says “Wilson High School”) Along with the work permit form, we need a copy of your social security card AND a copy of your Birth Certificate OR Passport. You will see places to upload those documents when you log-in to CampBrain.
