Acknowledgment of Risk and Agreement to Follow COVID-19 Protocols
The following form must be filled out by families prior to attending in-person camp. While this is just a copy of the form, we want to provide families an opportunity to become familiar with it prior to signing it while registering online.
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
By signing this form, affirms having read, and hereby acknowledges and agrees to the following terms and conditions:
TIC Summer Camp (“TIC”) has put in place preventative measures to reduce the spread of COVID-19, in accordance with Federal and State recommendations and in consultation with County Health Department officials, American Camp Association and CDC; however, I acknowledge that TIC cannot guarantee that my child(ren) and anyone in contact with my child(ren) will not become infected with COVID-19. Furthermore, attending the camp could increase the risk to my child(ren) and to myself of contracting COVID-19.
I acknowledge the contagious nature of COVID-19 and the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the camp and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand and willingly accept and assume full responsibility for all risks, damages, liabilities, losses and/or expenses of any and every kind involved with or arising from my/camper’s participation in camp. I further understand and agree that this Agreement includes any Claims related to a COVID-19 infection, whether such infection occurs before, during, or after appearing or participating in TIC.
I also hereby certify that I have read and understood TIC’s COVID-19 protocols, shared with me and posted on TIC’s website.
I agree to abide by these protocols, including, but not limited to:
responding honestly and accurately to the screening questions each day of the 2021 summer season during which I have a child enrolled in a program at TIC;
to keep any symptomatic child at home, as stated in and according to the protocols;
to notify the Camp Director or Health Advisor if my child has tested positive for COVID-19 or has been in close contact with someone who has tested positive for COVID-19 up to 14 days after attending TIC Summer Camp;
to send with my child each day an appropriate mask that fits him/her/them properly; and
to instruct my child to follow the camp’s protocols for wearing the mask, washing hands, and staying socially distant, according to directions given by TIC staff.
I further agree:
To allow staff to check on my child periodically during the day, including taking their temperature;
That if my child shows symptoms of COVID-19, TIC staff will escort them to a separate isolation area on campus away from the group, but under supervision, until he or she can be picked up;
That if my child is symptomatic I will come personally or arrange for another responsible adult to pick that child up as quickly as possible, within the hour, after being informed of my child’s symptom(s).
If the camper becomes symptomatic, the family agrees to the proper quarantine or procedure as advised by a health care professional.
Before & After Your Session
Before your Session (Pre-Session Screening):
Precautions should be taken, when possible, to limit outside exposure prior to attending camp. We suggest campers not travel or participate in high-risk activities outside of camp in order to minimize any potential exposure to COVID-19, as well as doing the following:
Self monitoring for 14 days prior to attending camp, including taking and recording your camper's temperature in order to know your camper’s typical temperature, as well as self screening campers for the presence of symptoms.
Determine if, within the past two weeks, your camper or a member of your household has traveled and may have been exposed.
Determine if your camper or a member of your household has been in close contact with a person who has been diagnosed with, tested for, or quarantined as a result of COVID-19.
Prior to the arrival/check-in of each scheduled session, all participants and families will receive an automatic email that provides the most up-to-date version of the applicable Assumption of Risks and Indemnity Agreement that all participants agreed to during registration. This contains some important explanations of inherent risks related to COVID-19 and will also include the following affirmation from each participant. I affirm that, within the last 10 days, my camper has not:
Had a new fever of 100.4 or higher, or a sense of having a fever.
Developed a new cough, congestion, runny nose or loss of taste and smell that cannot be attributed to another health condition.
Developed shortness of breath that cannot be attributed to another health condition.
Developed a new sore throat that cannot be attributed to another health condition.
Experienced muscle aches that cannot be attributed to another health condition, or that may not have been caused by a specific activity such as physical exercise.
Developed nausea, vomiting, or diarrhea.
Traveled within an area identified as a COVID-19 “hot-spot.”
Been in contact with an individual who has been ill with respiratory complaints or fever, or who I know has tested positive for COVID-19.
Been diagnosed with COVID-19.
If my camper or a member of my household has tested positive for COVID-19, within the last 14 days, I agree to disclose to the organization the date on which they were notified that they were no longer contagious with COVID-19. Documentation may be requested.
If my camper or someone in my household has tested positive for COVID-19 antibodies, within the last 14 days, I agree to disclose that fact to the organization. Documentation may be requested.
After your Session:
If your camper, or a household member, has a suspected or confirmed case of COVID-19 within 10 days of their Session ending, please let us know right away by emailing Emily Riedel at emily@ticcamp.com.
